Thank you all for keeping this going! It is such a great tribute to Ron. I also (like so many others have said) miss him dearly- he was a great mentor. The references are very useful, but the one feature I loved was the original article or editorial had an embedded link to the full article. It was quick and easy to refer to the full manuscript... is it possible to bring back this feature? Thank you again!

Thanks to Drs Yaster and Elliot. Definitely an important topic. Needs ongoing evidence based discussion. I’ve left a link to this at https://pedsanesthesia.net/wiki/index.php?title=Preoperative_Fasting and hope the wiki could be a place for both solid info and discussion…Ron’s wish.

Drs Yaster and Elliot, thank you for writing on this topic. After leaving CHOP I have struggled greatly with NPO policy at my places of employment in trying to relax them or push for adoption in the adult world for more ERAS protocols. Sadly, until national standards are revised many of my colleagues will not be comfortable with these changes. I have heard of suit after suit(adult and pediatric) where NPO time is challenged, questioned, or pinned as the primary reason there was a bad outcome when laryngospasm can and will occur by many other means. This scares many in our field to simply cancel cases without thinking through the consequences for all parties involved. Hopefully the meaningful discussion at the national level will move the barometer in the right direction. I applaud your insight and that of the teams doing the research to build on this topic!
